Co-Pays and Insurance
Your insurance card is required the day of your appointment so we can bill your insurance correctly. Co-pays are collected at the time of your visit as required by your insurance plan. Please be prepared. A picture ID is required at check in.
We are contracted with mostly all Insurances.
Non-Insured Patient
If you are paying for your own healthcare $75.00 is requried at the time of your visit. If surgery is required we require half down prior to the surgery. The surgery scheduler will go over payment arrangements.
Referrals
For non-emergent care your insurance company may require a referral. As a patient obligation we ask that you have that sent to us prior to your appointment or you may bring a copy with you on the day of your appointment. If we do not receive your referral your insurance company will not pay your full benefits and the doctors often will not get paid.
