Thank you for choosing Monroe General Surgery and Puget Sound Vein Center. We are committed to providing the best care. Please understand that paying your bill is part of your treatment. The following explains our Financial Policy.
Participating Health Insurance Plans
Monroe General Surgery and Puget Sound Vein Center are contracted with most major health Insurance plans. If you are not sure that we are a participating provider, please contact your insurance carrier (there should be a customer service number on your insurance card) to inquire that Michael S. Eickerman is a participating provider. You may also contact our office.
Insurance Processing
As a courtesy, we will process and file your insurance claims. We require that the guarantor, the person who is financially responsible, is personally liable for all balances not covered by including applicable co-pays for each visit. It is your responsibility to understand and comply with any predetermination of benefits or referral requirements. In the event of non-coverage, the guarantor is personally financially responsible for any non-covered service feesntor is personally financially responsible for any non-covered service fees.
Referrals
If your health plan requires a referral in order to receive care, it is your responsibility to make sure an appropriate referral has been requested prior to your visit. Typically, your Primary care doctor will begin this process if they are the referring physician. Some health plans require a referral for each visit, others do not. If you do not obtain a valid referral for you primary care physician, and your health plan does not pay for our services, you will be responsible for payment.
Co-Pays
Payment for Co-pays is expected at the time services are rendered. The fee is not covered by your insurance, therefore, it will be the guarantees personal obligation.
Non-Insured Medical Services
If you do not have insurance coverage, you will be asked to pay for your visit at the time of service.
Non-covered Medical Services
For services not covered by insurance, the practice requires 100% payment of the total charges prior to treatment.
Past Due Accounts
Overdue accounts will be referred to a collection agency. Any legal fees that we pay to secure past due balances will be applied to your account. We realize that temporary problems may affect timely payment of your account. If such a problem arises, we encourage you to promptly contact for assistance in managing your account. Our staff will assist you in arranging monthly payments over an approved term. Please do not hesitate to call us.
